Tailored Care Management (TCM) connects eligible Medicaid members with a specially trained care manager who helps coordinate healthcare, navigate Medicaid services and connect you with resources and support.
Tailored Care Management is designed to make it easier to coordinate the different services and resources that support your health. At Blue Ridge Health, your Tailored Care Manager can meet with you at a BRH location or in the community to get to know you, understand your needs and help you identify your goals.
Your care manager can help connect you with doctors and behavioral health providers who accept Medicaid, assist with appointments and transportation, connect you with food and housing resources, help you manage chronic health conditions, and find support related to work or school.
Your Tailored Care Manager can also serve as a central point of contact for your Medicaid services and advocate for additional services and support when you need them.
