Patient Rights:

  • You have the right to receive information from health providers and to discuss the benefits, risks, and costs of appropriate treatments

  • You have the right to a relationship-based, Primary Care Medical Home that helps you organize and manage your care at the level you choose

  • You have the right to choose your healthcare provider

  • You have the right to courtesy, respect, dignity, responsiveness, and timely attention

  • You have the right to confidentiality

  • You have the right to continuity of healthcare

  • You have the right to have oral and written communication needs met in the manner in which you request

  • You have the right to refuse identification photography for purposes of the electronic medical record

Patient Responsibilities:

  • You have a responsibility to provide complete medical history, to the extent possible

  • You have the responsibility to request information or clarification about your health status or treatment

  • You have the responsibility to cooperate with the treatment plan that you and your health provider agree upon

  • You have the responsibility to behave with respect towards providers, staff, and other patients while at a Blue Ridge Health facility

  • You have the responsibly to pay for services (your insurance co-payment, the Blue Ridge sliding scale fee, or full payment) at the time of you visit. If you have questions about how to pay or would like to pay your bill click here.

  • You have the responsibility to pay the remaining balance on your bill after Blue Ridge Health has successfully filed for reimbursement with your provided insurance carrier – including private health insurance, Medicare, and Medicaid

  • You have the responsibility to comply with the application requirements if you are eligible and would like to apply for an extended payment plan or discounted services based on a sliding scale. This includes a yearly re-qualification process

Blue Ridge Health Policies:

  • Blue Ridge Health does not discriminate in the provision of services to any individual based on (i) the patient’s ability to pay, (ii) the patient’s health insurance payment source by Medicare, Medicaid, or the Children’s Health Insurance Program (CHIP), or (iii) the patient’s race, color, sex, national origin, disability, religion, age, or sexual orientation

  • Blue Ridge Health does not provide chronic pain management services

  • Blue Ridge Health prohibits the use of camera phone recording devices during clinic visits without the prior approval of the service provider

  • Blue Ridge Health does not allow weapons in the buildings (leased or owned) or on a property where Blue Ridge Health services are provided

  • Blue Ridge Health reserves the right to reschedule your visit, refer you to another practice, or dismiss you from our practice should a patient fail to comply with the Patient Rights and Responsibilities outlined above and in the brochure below:

Innovative Ways Blue Ridge Health Can Keep You Healthy During COVID-19

During this cautious time, Blue Ridge Health remains a trusted source for all of your healthcare needs. We encourage patients to use virtual visits that are now available to take care of your medical and behavioral health care needs.  We’re here to help keep you healthy no matter how long the pandemic lasts.

Due to the Coronavirus/COVID-19 outbreak, patients visiting our practices should anticipate the following changes:

  • Please bring a mask or cloth face covering with you to your visit, if possible. 

  • When you arrive at the practice, look for a member of our staff to provide site-specific instructions.

  • If you do not see a staff member, call the practice and instructions will be provided.

If you suspect that you have been exposed to Coronavirus, or you have a fever, cough or shortness of breath, please contact your local Blue Ridge Health office prior to visiting. They are happy to help and will provide further instructions.

Please be aware that these changes are necessary in order to serve you best.
