New Patient Process

Welcome to Blue Ridge Health!

We are excited to introduce you to your new medical home. Here’s what you need to get started:

  1. Choose a Provider - browse our list of providers and locations to find the best fit for your care.

  2. Schedule an Appointment  – contact us to schedule your new patient appointment. If a provider’s schedule does not work for you, our Patient Support team will assist you in finding the right provider and confirming an appointment time for you.

  3. Complete your Paperwork - You will need to complete new patient paperwork before your first visit with us. If you would like fill out these forms before the day of your appointment you can access them at the links below:

  4. Please arrive 15 minutes prior to your appointment time with any paperwork you have completed, a photo ID, and your insurance card.

We accept private insurance as well as Medicaid and Medicare. No insurance? No problem. Sliding scale fees are available for those who qualify and we can help you apply for coverage.
