NC Health Information Exchange (NC HIE)  

To our Patients:

All Blue Ridge Community Health Services, Inc. sites are participating in a statewide information-sharing network that will improve the health care you receive.  This new technology is being used statewide and helps doctors and other health care providers share important patient health information. It will help us more easily and quickly share your health information with other health care professionals caring for you. This can improve the safety and quality of your health care.

This new technology is available to us through our partnership with an independent nonprofit called NC Health Information Exchange (NC HIE). NC HIE combines information from separate health care sites to create a single electronic patient health record. This record combines health information from all your health care providers and allows authorized health care professionals to see it when caring for you. Your personal health information and privacy is very important to us. NC HIE takes many precautions to keep your records private, and your NC HIE record can only be viewed by providers who are directly involved in your care.  The information that can be viewed is listed below:

Information included in your NC HIE record: 

Medicines (prescriptions), allergies, lab and test results, image reports, conditions, diagnoses, and vaccination history.

Your name, birth date, address, sex, phone number, and the last four digits of your Social Security number to make sure your health information is entered into the correct record.

Information not included in your NC HIE record: 

Information documented by a substance abuse treatment facility

While we believe the use of secure networks like NC HIE will improve the care you receive, you can choose to opt-out of having your information shared through the health information exchange network by following the instructions on the NC HIE Opt-Out form below.

HIPAA NOTICE OF PRIVACY PRACTICES

We respect the confidentiality of your relationship with your doctor and other caregivers, and the sensitive information about your health and health care that are part of that relationship. State and federal laws and our operating policies protect the privacy of your medical information. You will receive a BRCHS Notice of Privacy Practices that describes the ways that we use, disclose and safeguard patient information and that explains how you can obtain a copy of information from our records about your care.

Being a good patient does not mean being a silent one. If you have questions, problems, safety concerns, or unmet needs, please let us know.  CORPORATE COMPLIANCE

BRCHS expects its patients, staff, or any other individual affiliated with BRCHS to report concerns, suspected violations and/or irregularities of the Compliance Plan, Code of Conduct, laws, rules or regulations, or any identified issues or questions associated with BRCHS's policies or procedures with respect to any Federal health care program (e.g. Medicaid, Medicare). It is the policy of BRCHS to provide a procedure through which our patients, staff, or other any other individual affiliated with BRCHS may confidentially report their questions or concerns without fear of retribution or retaliation.

If a patient, staff member, or any other individual affiliated with BRCHS desires to remain completely anonymous, the individual may submit his/her report through the BRCHS Corporate Compliance Hotline (828) 233-2238. No adverse action or retribution of any kind will be taken by BRCHS against any patient, employee, or any other individual because he or she reports in good faith any concerns or suspected violations.

 

JOINT COMMISSION

BRCHS is accredited by the Joint Commission.
